There are days when panic creeps into Kathy Needleman's voice. It was there one November evening, just after the police had left her Concord home. She leaned back in her living room chair and covered her face with her hands.

Kathy Needleman describes her son's disorders like this: He is an 80-story building built on an unstable foundation. The foundation represents his early years, when 10-year-old Jacob lived in an orphanage in Russia.
